CIGNAL APPLICATION REQUIREMENTS

• 1. Proof of Identification (Driver's License, UMID, PRC etc with your photo)
• 2. Proof of Billing within 3 months of the date of the application (Meralco, Credit Card Statement, Mobile Postpaid Billing Statement, PLDT, Cable or Internet Billing Statement) Any of the docs must be under the applicant's name.
• 3. Signed Cignal TV Service Application Form (SAF) To be  provided by the installer
