2. Proof of Billing within 3 months of the date of the application (Meralco, Credit Card Statement, Mobile Postpaid Billing Statement, PLDT, Cable or Internet Billing Statement) Any of the docs must be under the applicant's name.
3. Signed Service Application Form (SAF) To be provided by the installer
